Chronic Care

I believe that a personalized, supportive doctor-patient relationship is the foundation for optimal care. Toward that end, I provide the much-needed support and advocacy that my patients and their families need in order to manage the medical changes and the social determinants of their chronic health conditions.

I offer services for a variety of chronic medical conditions, including but not limited to hypertension, diabetes, hyperlipidemia, heart failure, cardiovascular disease, asthma, depression, and other common conditions.

Acute Care

Things come up, that's why I'm here. Whether it's something that we can manage in the office, through a home visit or over a telehealth appointment I am here for you. If your condition is more serious, I am your ally in navigating through our complex health care system and can assist in getting you to who you need to see and where you need to be. I also have privileges with El Camino Hospital Network and can help coordinate your hospital care in the event that you require hospitalization. Regardless of where you are hospitalized-if this does occur, I find it important to at least communicate with the primary team so that I am as up to date as possible with your health. It is also very important that if you visit an urgent care, emergency department or become hospitalized that your notes, imaging, and lab data get faxed to: (650) 860-1166 by the provider taking care of you.
